If you haven't destroyed three ladders when you locate the curving sewer section, you've missed ladders near where you dropped into the stage. The curving sewer section is one of the harder parts since it is directly vulnerable to an MG42 overlooking the whole tunnel. To defeat it, you need at least one grenade (or a lot of Kar98 ammo and fast reflexes) and be on the lower level of the sewer.
